Here is a modification of a program that worked for me.
It searches a directory for.doc files, opens them in word and then saves. Then i used saved as to. Publish what, click on the document showing markup. Before converting my word document to a pdf i made sure everything microsoft word flagged in the accessibility check was fixed.
In word use the save a copy, change to pdf (*.pdf) > click on more options, new menu will open up. That included adding alt text to my images. Adobe acrobat dc, as well as several other pdf creation products (foxit, nitro, nuance) use macros that will interpret the styles and the navigation tree in word to create bookmarks and set.
